As the person submitting this request, you are the group leader and single point of contact for this visit - please read the terms below and sign on behalf of yourself and your party. You must be 18 or older to do this.
Terms and Conditions1. Assumption of Risk: I, the undersigned, understand that by entering the premises of RESQ Charitable Trust, I may be exposed to risks associated with wildlife, animals under rehabilitation, and other activities conducted on-site. I voluntarily assume all risks associated with my visit, and with the visit of the party I am bringing with me.
2. Waiver of Liability: I agree that RESQ Charitable Trust, its management, employees, volunteers, and affiliates shall not be held responsible or liable for any injury, loss, damage, or harm (including but not limited to physical injury or property damage) that may occur during the visit, whether caused by negligence or otherwise.
3. Compliance with Safety Protocols: I acknowledge that I, and everyone in my party, will adhere to all safety protocols, instructions, and guidelines provided by RESQ Charitable Trust staff during the visit. I understand that failure to comply may result in the termination of the visit and immediate removal from the premises.
4. Indemnity: I agree to indemnify and hold harmless RESQ Charitable Trust, its trustees, employees, volunteers, and affiliates from any claims, actions, damages, liabilities, or costs arising out of my actions or the actions of my party during the visit, including those resulting from negligence or non-compliance with instructions.
5. Confidentiality: I agree not to disclose any sensitive or proprietary information about RESQ Charitable Trust, its operations, or its animals to any third party without prior written consent. I also agree that neither I nor anyone in my party will take photographs or videos on the premises without explicit permission. If found doing so, I agree to hand over the device upon request for the deletion of any unauthorised images or footage.
6. Permission for Media Use: I understand that RESQ Charitable Trust may take photographs or videos during the visit. By signing this form, I grant permission for my likeness and that of my party to be used in any RESQ Charitable Trust publications, social media, or promotional materials, unless I explicitly state otherwise.
7. No Claims: I understand that this is a charitable facility, and that I and my party are entering as visitors. I waive any claims to services, compensation, or benefits in connection with the visit.
8. Responsibility for the Party: As the person signing this form, I confirm I am the point of contact for this visit and take responsibility for everyone in my party for the duration of the visit, including their compliance with the terms above.
Declaration: I, the undersigned, have read, understood, and voluntarily agree to the above terms and conditions on behalf of myself and my party. I accept the risks involved in this visit to RESQ Charitable Trust and agree to adhere to all safety measures and protocols during the visit.
